In honor of Alfredo Marte's pinch-hit grand-slam last night, how many of the other 58 grand-slams in D-backs history do you remember?

For each slam, you get the date it was hit, and as extra help, you get the players who scored ahead of the slammer. You only have to enter a player's name once, to get credit for all their slams (one player has an impressive seven, no-one else has more than three), and last names only are fine.
